Appointments
Because
we do not accept walk-ins, please call the office to make an appointment.
We require 24 hours notice of cancellation or reschedule; failure to do so will result in a $25 penalty. We make every effort to adhere to our appointment
schedule and appreciate your patience in the event that we are late
due to emergencies or hospital surgery.

Fees & Insurance
Our
fees are based on what is usual and customary for this area. Payment
for services rendered is expected at the time of service. As a courtesy
to you, we accept Medicare assignment and most insurance plans and
will bill your insurance company. However, you will be responsible
to pay for any co-pay, coinsurance and/or deductible on the day
of your visit. Payment arrangements can be made by calling our business
office at (877) 269-8434. You remain responsible for any unpaid portion of your bill.
Please ask our office staff if you have any questions about our
fees or to confirm our participation in your insurance plan.
Prescriptions &
Renewals
Please
request all prescriptions and authorizations for renewals between
8:30am - 4:30pm, Monday to Friday, when our full records are available.
